Description : The otoscopic appearance of SOM depends in large part upon the amount of fluid, and the degree of negative press within the middle ear.

The straw colored transudate within the middle ear gives the drum a color which varies from golden yellow to orange.

When Eustachian tube function starts to return and the middle ear begins to reaerate, air bubbles or an air-fluid level may be seen.

Unlike MOM, the tissues of the drum are usually nomal and there is no dilatation of the tympanic vessels.

When the level of negative pressure within the middle ear is high, the tympanic membrane will be retracted, with the handle of the malleus appearing foreshortened and the lateral process of the malleus appearing unusually unusually prominent.
