Histologically, tube granulomas combine the features of acute inflammatory granulation tissue (numerous newly formed capillaries and an inflammatory polymorphonuclear leukocyte infiltration within a loose, areola fibrous tissue stroma) and the features of a keratin foreign body granuloma.
The key to the histologic diagnosis of keratin foreign-body granuloma is the presence of keratin squames within otherwise empty and irregularly shaped slit-like spaces which are surrounded by foreign-body giant cells. The keratin squames can be identified by their shape, size, strong eosinophilic staining, and by birefringence under polarized light. |
